Middleton Family Medicine Middleton Family Medicine Middleton, MA Middleton, MA Part-time Full-time Part-time Full-time $60 - $80 an hour $60 - $80 an hour Nurse Practitioner Adult/Gerontology (Part-Time or Full-time) Middleton Family Medicine is seeking a passionate, motivated Family Nurse Practitioner who will work hard to ensure our patients receive exceptional medical care at our growing House Call Program.
This is a critical position that requires a passion to provide quality health care at our patient's homes.
Selected candidates will work in a collaborative team that recognizes the patient as the central focus and serves as a patient educator in areas of specialization.
Our Nurse Practitioners/PA's support the supervising physician in patient situations in which physical assessment is required as well as complex communications with patients.
Must be able to assess, diagnose and treat complex illnesses.
Other duties include, but are not limited to, providing telephone evaluations and advice to patients, monitoring and managing chronic disease cases of moderate complexity, participating in department initiatives, and serving as the physician back-up.
Middleton Family Medicine House Calls Program is open to part-time or full-time applicants.
Required days are Wednesdays, Fridays and vacation coverage.
This position requires a candidate with outstanding critical thinking skills, excellent verbal and written communication skills, computer and EMR proficiency, the ability to work both independently and as part of a team, and a commitment to doing what's best for our patients.
Essential Job Duties and
Responsibilities:
Performs new patient screenings, conducts yearly physical examinations, obtains patient medical histories, confirms and updates patients' medical concerns Updates patient medical records and confirms accuracy of information included in the chart at each patient appointment.
Orders, interprets, and evaluates diagnostic testing to identify and assess patient clinical problems and healthcare needs Reviews lab, x-ray, and ED or Admission reports and initiates appropriate patient follow-up to ensure the best long-term health outcomes Prescribes medications and other forms of treatment such as physical therapy, respiratory therapy, and occupational therapy to appropriately treat patients' acute and chronic conditions and improve quality of life Treats acute illnesses and diseases such as cold, flu, and infections Treats and manages chronic diseases such as high blood pressure and diabetes, with coordination of specialty teams when indicated Develops and implements treatment plan to address acute and chronic conditions Cooperates and collaborates with leadership team to implement operational initiatives that drive improvements in patient care and practice performance Educates patients about health risks, preventive care, diet, exercise, and overall health maintenance Collaborates with other healthcare providers and professionals in coordination of care and case management Performs all other related duties as assigned Knowledge, Skills, and Abilities:
